Faculty/Staff - Telecommunications Service Request (TSR)

In order to have telephone service in your office, you will need to fill out a Telecommunications Service Request (TSR) form.

Section 1 - Billing Information

• Fill out all parts of this section.
• Note: Be sure to obtain the Fiscal Officer’s signature. We cannot proceed without authorization.

Section 2 - Work Requested

• Indicate if we need to install a new jack or if one is already in place.
• Indicate building and room number.
• State your preference for a phone.
• Tips on Filling out a TSR

Section 3 - Description of Work Requested

• Use this section for a description of work requested and/or any additional details that may pertain to your request.
• You may mail the request to us via campus mail, mail code 4432, or bring it by our office, Student Center, Room 127E.
